Release Forms

In the event that you will need to leave your child with another responsible adult, or have them pick your child up, you will need to fill out the following forms and give them to your director.  You may NOT leave your children in the care of another adult without a Child Release Form and a Medical Release Form on file.

Every parent that is on campus is REQUIRED to have a Parent Emergency Contact Information form on file with the director.  Should anything happen to you, we will need to know who to contact.  Please be sure you hand this in on the first community day.
